I cannot provide an URL as of now, because it has some sensitive information from my firm, and is password protected. I will provide a live example if I don't find out what causes this any another way.
I might've been a little unclear in my first message though, it is only the code tag itself that disappears, not the text (in this case SQL code) it contains.
Here's an example (edit page):

Note that the
<pre class="code">[/m] is located in the wrong place, it should be before [m]CREATE TABLE, additionally no SQL highlighting has taken place.
I might add that I get no PHP errors or warnings in my httpd log from this.
The server is a Fedora Core 5, Apache 2.2.2, PHP 5.1.4.
