Thanks for the information! I have muddled through a few issues of my own since getting started with DokuWiki about a year ago. I'll warn you up front that I know very little about hosting on Windows, but hopefully it won't be a Windows-specific issue.
First, let's try to eliminate the possibilities listed in the FAQ:

Using PHP 5.3.0: you are now using PHP 7, and you were on PHP 5.5, so it's not that.

Style not loading after adding/removing Plugin: you say you experience the issue with a totally fresh installation. How are you uninstalling/reinstalling DokuWiki? I wonder if some remnant of a prior installation or cache or something is the cause here.

Incomplete styles (Ponder Stibbons release): you're working with the latest release (Frusterick Manners). F-Manners is considered "Stable (recommended)" now, and I'm not aware of any compelling reason to start a fresh install with an older version of DokuWiki. The FAQ implies that this particular cause of style issues will have been solved in the Hrun release in September 2014, although I don't know for certain that it was. Still, most people aren't experiencing this issue, so chances are good that it's something specific to your installation.

Styles not working: this sounds like what you're experiencing:

I'm not sure why your stylesheets would be broken with a totally fresh installation - it would be interesting to know what steps you took to ensure that your recent attempts weren't contaminated with remnants of a previous installation - but for sure it is worth checking out your stylesheets carefully. There are five steps listed in that section of the article:

1. Disable compression/compaction: as above, probably not an issue with latest DokuWiki, but then again, why not try it briefly?

2. Cleanup the cache: clean out the server-side cache. Definitely worth a shot. The cache will be rebuilt as you use the wiki.
*
3. Clear the browser's cache. Standard stuff, but might help.
*
4. View CSS separately. This could be a useful way of finding out what's wrong. You could even try feeding the result into the W3C's CSS validator, although be warned that almost every website you've ever been to will cause the validator to show some sort of minor warnings, but anything big flagged up there could be the source of your trouble.

5. Disable plugins. Again, I know this is a fresh install, but if you've run out of ideas, see if you can track down the problem this way.
Are you able to try these suggestions in order and report back? I will do my best to help you if you run into issues.
I'd also recommend checking out Developer Tools in your browser (e.g. F12 in Chrome). Perhaps your browser will report trouble loading a stylesheet or other required resource.
What happens if you enable DokuWiki's
debugging mode? You might get to see some extra error messages on wiki pages. Additionally, some extra logs are created on the server.
Talking of logs on the server, do you have access to the logs of the webserver itself? I have sometimes found useful hints in my Apache logs, but I know not every hosting provider makes them available.
Lastly, is the wiki public? Screenshots are very helpful, but it would be useful to see the issue live if possible.
