This page may be of use to you.  It doesn't provide a specific solution, but may give you info that would help to create said solution.

You will probably also want to look at the metadata discussion page, and the indexing page, which are both linked from there.

I do not believe you can add an image directly.  Using the  tags you can link to some image online somewhere.  If you click on Post Options while posting a reply, you'll see an option to upload a file, however I do not believe this will appear inline.
Sure it does, look at the showcase thread. You have to save your post in order to upload the pic.
Hmm, there are no images shown inline on that thread that I can see, which is what I thought the OP was going for :)

Izzy, can you give a link to a page where the code tag is not working correctly?

To my knowledge this is not possible with dokuwiki out of the box.  However  you may be able to get close to where  you want by creating some blank cells inbetween, or perhaps creating empty header cells inbetween.

Have you tried 'touch style.ini' in the template directory?

We have one 'production' wiki up (with a couple of others for testing of new patches and our own work), set up on our internal network.  Until recently it was the standard template, with some modifications to integrate it a bit better with some of our other applications.  I decided to take a break from 'real work' recently and change the looks, which ultimately ended up with usage of the ACH template with a stylesheet to better match the look of above mentioned apps.  Documentation used to be sporadic and difficult to find around here, spread out between random notes in different applications, random paper documents lying on various desktops, and mostly random text files on various servers.  Having a home for all documentation, and having it exist in a format that is fairly easily exported to any other format we may want for a particular case is a great time saver.

The look is nothing very special, but in case anyone wants to see a screenshot is attached.
