I updated to the current bootstrap and did some tests.  I added two rules to the end of your userstyle.css file:
h1 { font-style: italic !important; }
h2 { color: #FFA500 !important; }
The result is here.

In other words, it accepted my two rules but rejected yours.

When applying the same css to the dokuwiki template, I get the following:

Here it is a combination of your css with my additions.  But there is something skewed here, because the header 3 is shifted to the right, and I don't think that is what you intended.  So I suspect there is something in your css which is not correct and/or conflicts with the bootstrap css.

If you have admin rights, then  you should be able to use the extension manager.  If not and you have rights to the server and to the dokuwiki files, then:
In conf/plugins.local.php add the plugin you wish to disable:
$plugins['plugin_name'] = 0;

If conf/plugins.local.php does not exist, then:

$plugins['plugin_name'] = 0;

It did make changes but not what was expected.  But it did have an effect.

Did you clear out data/cache?  I had to clear out data/cache to get back to where the template was before inserting your code.

\my point is this:  if your own styles apply to the bootstrap template and if the update to the template has changes that affect your styles, then maybe that is the problem. 

Could you attach your userstyle.css?

My point was if the settings in userstyle.css apply specifically to bootstrap3.

If the userstyle.css settings apply to the template, it's possible that the update has changed the css.

Try refreshing your browser cache and, if necessary, deleting the cache directories in data/cache.

The faulty sample epub has been replaced:  https://www.mturner.org/devel/epub_sample
The problem with the original sample is that the one of the pages referenced in the markup had been deleted when I rebuilt the ebook.  This didn't show up as a problem in either Firefox or Calibre.  I'll have to look into this.

Dokuwiki is the fallback for templates that do not support mobile. Read the documentation. Obviously the OP did not.
