Ckgedit will work with both internal and htaccess, so something else must have happened to cause the problem. But I'm glad you could get it working.

systems differ.  Check google for your system.  On some linux machines,  it's in var/log/httpd, which is the apache log directory. Usually errors are written to the apache error log.  On other linux machines, like Ubuntu it's in var/log/apache2.
You'll have to check for windows yourself.

I have no solution for you, because there's not enough to go on in your description.  Does the plugin load? Or does it not show up in any form, i.e. no ckgedit buttons when the editor opens, no ckgedit toolbar?  In the meantime clear out the dokuwiki cache (all directories in data/cache) and refresh your browser cache.

my ACL Rules Settings is @ALL 1 (read)
but must be logged in to read the page.

This obviously doesn't make sense, since whatever you've done to restrict reads to users disables @ALL 1.

You can adjust the size, both vertical an horizontal, with the arrows at the lower right corner.  Or simply by grabbng the lower right corner.i

I did a quick check.  The toc key returns true if the page can display a toc, not if it currently has one.  So, if you add the ~~NOTOC~~ macro to the page, then the value for the toc key will be false.

Attached is a script that will read meta files (and as a bonus) unpack .gz files.  It will have to be unzipped.

You use it from the command line: 
[root@dirname meta]# get_inf.php <metafile.meta>


If your php binary doesn't happen to be in the default directory you will have to use: php get_inf.php

This may just be a caching problem  Refresh your browser cache and delete all directories in data/cache.

I am not aware of any plugin that creates a 2 factor login, if that is what you are looking for.  But you might be able to use the captcha plugin.

Afraid I didn't read your post carefully enough, and didn't take into consideration the fact that this is not just a logo problem but media problem generally.  Still, I would first try refreshing the cache in both browser and your dokuwiki.  And then I would try disabling nice urls, on the chance that the configuration for nice  urls has been corrupted.  And also I would try reverting to the dokuwiki template, to see if it is something in the bootstrap3 template that is causing the problem. 

Have you recently in any way made changes to your dokuwiki, either with a new plugin or by updating a plugin?  And have you in any way made changes to the dokuwiki code?

There is no folder named "media"; it is a regular expression placeholder for a call to "lib/fetch.exe. . .".  It is used for for the .htaccess level of nice urls. So, "_media" as a directory is never accessed.  So, first, refresh your browser cache and delete all the directories in data/.cache.  

If that doesn't help, disable nice urls and see if the logo appears.  If not, then revert to the dokuwiki template.  If the logo appears then there is some glitch in bootstrap3 or else bootstrap3 has a requirement that you haven't met.   Typically, the logo should be in either the templates images directory or the media directory.

Make sure it's enabled in the Extension Manager.  If for some reason it has been deleted, make sure you have the latest dokuwiki, and then download a separate  copy of dokuwiki and in lib/plugins you will find authad.  Copy its whole directory to lib/plugins and make sure it's read-write to the web server.  To get the latest dw, if you don't have it, use the upgrade plugin; consult the dw wiki for upgrade instructions.
