I wanted to post of original response to
this thread, but after doing some searching I thought I would post this new topic and link to the others as sort of an overview.
This is for solving the "User addition failed" error message.
Here are the related threads that at least mention the same error I encountered, with possible different causes and solutions:

Using Dokuwiki 'Release 2015-08-10a "Detritus"'.
If someone else happens to be trying to add users like I was in a weird way, I found another error case that results in the same error message that lacks enough detail to properly debug by one's own self. Hopefully this will be improved soon by this issue:
https://github.com/splitbrain/dokuwiki/pull/1381
I checked the bundled plug-in "User Manager"'s page here:
https://www.dokuwiki.org/plugin:usermanager
I ran through the "?do=check"messages; my user directory was writable, so no problem there.
The problem was with my situation, I was adding users to a private Dokuwiki installation, and did not want to add real names. This must be a required field, and was probably failing because it was left empty. "Groups" may be left empty, as per the note below the form fields box, since that has a default value of "user", as well as the "Password" fields, but I'm guessing the rest require a value.
This should be denoted required, as I have already spent too much time figuring out why it wasn't working. I have opened this issue to track the request:
https://github.com/splitbrain/dokuwiki/issues/1388
Also, if anyone has the information that would like to leave a note about this: I was not able to find good Dokuwiki logs to find more detailed error messages. I tried accessing the lighttpd installation's "error.log" file, along with using debug mode and do-check as I mentioned earlier.
