I attempted to install and use the docimporter plugin, but I could not get it to work correctly. So, I gave up and uninstalled the plugin and reverted the changes I had made to lib/tpl/dokuwiki/tpl_header.php. Subsequently, I noticed that the images for the edit buttons on the right hand side of each page were missing. I didn't pay it much attention and decided to move on to trying the Vector template. I liked it, but when I tried to add my own logo, it came up blank. Text and the general appearance of the template are otherwise fine. My server error log showed the following:

I went through the log to back when the problem with the default DokuWiki template edit buttons went missing:

In each case there are additional lines in the log for each image for the templates. Any idea how this could have happened and what I have to do to fix it?
UPDATE 2016-08-03: I got fed up and reinstalled the server. No more problem. Don't install the docimporter plugin.
