So I thought all I needed to do was edit the mediamanager.php in my template folder, but it appears I was wrong. My wiki continues to use the default one; I guess it's pulling it from somewhere else.
How do I make dokuwiki use my version of the media manager so I can change the layout/css?

Change the layout using the mediamanager.php in your /lib/template/$whatever directory. Probably best to create your own version of the original template if you are going to do that.
Change the code that it uses in /lib/exe/mediamanager.php (not recommended unless you are certain you know what you are doing).
Change the style using the style files for your template - or in /conf/userstyle.css.

Did a bit more digging. It appears that the /lib/tpl/<whatever>/mediamanager.php only changes the media manager popup, ie what comes up when you click the "add image" button while editing a page.
I probably should have stated this in my original post, but I'm creating a template, so I want every change to be self-contained within the proper template directories. I've noticed that my page's css shows on the media manager page (probably because it's a php include overlayed on top of the base.)
If that's the case then I have a lot of work ahead of me, because HOOOO BOY does the media manager not like my css. The resizeable split-panel deciding to balloon to a length of over 17,000 pixels springs to mind. *shudder*
