Ok, based on your comments here I figured out how to fix my problem as well.
1. I have a 'forms' namespace by creating a folder in D:\xampp\htdocs\wiki\data\pages\forms
2. In the forms namespace, I created a page and copied the example from the
http://www.dokuwiki.org/plugin:bureaucracy
<form>
action template userstpl forms :
fieldset "Create Your User Page"
select "What's your Continent?" "Europe|North America|South America|Asia|Australia" @
textbox "What's your Name?" @
textarea "Enter a short bio"
submit
</form>
3. Created and moved userstpl.txt file into D:\xampp\htdocs\wiki\data\pages
====== @@What's your Name?@@ ======
I'm living in @@What's your Continent?@@.
@@Enter a short bio@@
4. Went back to the example form and entered the following data into the form:
Continent: Europe
Name: John
Bio: I live in London
Hit the submit button and got the message:
"The page has been created, follow the link to open it."
forms
- europe
- john
5. I think for Unix, you must have read,write permission in the D:\xampp\htdocs\wiki\data\pages\forms
