better do it one by one. some internals changed and we usually provide automatic upgrade functions for these but remove them a few releases later again.

* Download the version you had installed back then
* unpack into a separate directory
* create a diff between the clean version and your version
* upgrade your version to the next release
* reapply your changes and manually fix conflicts
* repeat til you reach the current version

That's an option of course. You could save you a lot of future hassle by implementing your changes as proper plugins this time.
